What the LDH Test (Lactate Dehydrogenase) measures
Lactate dehydrogenase is an enzyme found in almost every tissue, including the heart, liver, muscles, kidneys, lungs and red blood cells. When cells are damaged they release LDH into the blood, so a raised level tells a doctor that tissue damage is happening somewhere, but not where. For that reason LDH is almost always read alongside other tests. It is used to look for haemolysis, where red cells are breaking down faster than normal, together with bilirubin and a blood count; to follow some cancers such as lymphoma under an oncologist; and as part of the assessment of certain lung infections, liver disease and muscle injury. It was once used to diagnose heart attacks, but troponin tests have replaced it for that purpose. Sample handling matters with this test: if red cells break inside the tube after collection, LDH reads falsely high, so an unexpected result is sometimes repeated before any conclusion is drawn. Strenuous exercise in the day or two before the test can also raise it.

How to prepare, the sample, and when to expect the report
Preparation
No fasting is needed. Avoid strenuous exercise for 24 to 48 hours before the test, and tell us about any recent injury or muscle injections.

Blood, from a vein in the arm (serum).

Within 24 hours.
Reading the result
Reference ranges differ between laboratories, so your report lists the laboratory’s own range beside each value. A result outside the range does not always mean illness; ask your doctor to explain it.
