What the TSH Test (Ultrasensitive) measures
TSH (thyroid stimulating hormone) is made by the pituitary gland and tells the thyroid how much hormone to produce. It reacts to small changes in thyroid hormone levels, so it often becomes abnormal before T3 and T4 move outside their usual ranges. That makes TSH the usual first test when a doctor wants to check whether the thyroid is underactive or overactive. A raised TSH generally points towards an underactive thyroid, and a low TSH towards an overactive one, although illness, some medicines and pregnancy can also shift it. "Ultrasensitive" means the laboratory method can measure very low TSH values accurately, which matters when separating a truly suppressed TSH from one that is just at the lower end of normal. The test is used for screening, for yearly follow-up once a thyroxine dose is steady, and as a starting point when symptoms are unclear. If TSH is outside the expected range, doctors usually ask for T3 and T4 or a full thyroid profile before deciding on treatment.

How to prepare, the sample, and when to expect the report
Preparation
No fasting is needed. A morning sample is preferred. If you take thyroxine, give the sample before your tablet. Stop biotin supplements for 48 hours beforehand.

Blood, from a vein in the arm (serum).
Reporting time
Within 24 hours.
Reading the result
Reference ranges differ between laboratories, so your report lists the laboratory’s own range beside each value. A result outside the range does not always mean illness; ask your doctor to explain it.
