What the Thyroid Profile (T3, T4, TSH) measures
The total thyroid profile measures T3, T4 and TSH together, which is how thyroid function is usually assessed at first presentation. TSH comes from the pituitary and tells the thyroid how much hormone to make; when the thyroid underperforms, TSH climbs to compensate, and when it overproduces, TSH falls. T3 and T4 are the hormones themselves, and reading them alongside TSH separates a thyroid problem from a pituitary one and shows how far function has drifted. Thyroid disease is common in eastern India and in women in particular, and its symptoms are easy to attribute to something else — fatigue, weight change without a change in diet, feeling cold, hair thinning, irregular periods, low mood, a slow or racing pulse. A profile is also the standard check before and during pregnancy, since untreated hypothyroidism carries real risk, and for anyone already on thyroxine whose dose is being titrated.

How to prepare, and when to expect the report
Preparation
Fasting is not essential, but a morning sample is preferred because TSH follows a daily rhythm and runs higher overnight and early morning. If you take thyroxine, take the test before your dose. Stop biotin or multivitamin supplements containing biotin 48 hours beforehand — biotin interferes with the assay and can produce misleading results.

Should I take my thyroxine tablet before the test?
Take the sample first, then your tablet. A dose taken shortly before the draw raises the measured T4 and can make your dose look higher than it is.
