What the Thyroid Profile (T3, T4, TSH) measures
The total thyroid profile measures three hormones in one blood sample: T3 (triiodothyronine), T4 (thyroxine) and TSH (thyroid stimulating hormone). T3 and T4 are made by the thyroid gland in the neck. TSH comes from the pituitary gland in the brain and acts like a thermostat signal: when thyroid hormone levels drop, TSH rises to push the gland harder, and when they are too high, TSH falls. Looking at all three together helps a doctor tell an underactive thyroid (hypothyroidism) from an overactive one (hyperthyroidism), and judge how far the levels have moved. Thyroid symptoms are often vague and easy to blame on something else: tiredness, weight gain or loss without a change in diet, feeling unusually cold or hot, hair fall, constipation, irregular periods, low mood, or a fast heartbeat. The profile is also commonly checked when planning a pregnancy, during pregnancy, and while a doctor is adjusting the dose of thyroid tablets.

How to prepare, the sample, and when to expect the report
Preparation
Fasting is not required. A morning sample is better, because TSH is naturally higher at night and early in the day, and taking samples at a similar time makes results easier to compare. If you take thyroxine, give the sample first and take the tablet afterwards. Stop biotin (often found in hair, skin and nail supplements) for 48 hours before the test, as it can distort the result.

Reading the result
Reference ranges differ between laboratories, so your report lists the laboratory’s own range beside each value. A result outside the range does not always mean illness; ask your doctor to explain it.
